H1N1 Flu Clinics Rescheduled

Free H1N1 flu vaccine clinics in District schools have been rescheduled. The following information has been provided by the Forest Grove School District:

Saturday, Oct. 31 – Tom McCall East Upper Elementary School, 10 a.m.-3 p.m.
Tuesday, Nov. 3 – Cornelius Elementary School, 1-5 p.m.
Thursday, Nov. 5 – Forest Grove High School, 11 a.m.-5 p.m.
Tuesday, Nov. 10 – Neil Armstrong Middle School, 1-6 p.m.
Thursday, Nov. 12 – Harvey Clarke Elementary School, 3-7 p.m.
Tuesday, Dec. 1 – Echo Shaw Elementary School, 2-7 p.m.

These replace dates announced earlier that had to be postponed because of lack of vaccine. These dates are subject to change, based on availability of the vaccine. The clinics are for anyone in the priority groups identified by the Centers For Disease Control (CDC). Individuals do not have to bring doctor's notes or other documentation that they are part of a priority group. The priority groups are: Children & young adults ages 6 months to 24 years; Pregnant women; Health care workers; Emergency first responders, including police officers; Caregivers of children less than 6 months of age; and Adults ages 25‐64 with underlying chronic medical conditions (diabetes, asthma, etc.).

Please note:
There is no cost to receive an H1N1 vaccination.
Both the nasal spray and shot versions of the vaccine are expected to be available at the clinics. Info on Nasal Spray / Info on shot
Children age 9 and younger will need two doses of the H1N1 vaccine, given at least one month apart.
Anyone receiving an H1N1 vaccination must fill out a vaccination administration record (VAR) form. Forms will be available at the clinics. Schools also will send forms home with students.
For clinics held during school hours, students must bring a completed form, signed by a parent, in order to be excused from class to get vaccinated.
Parents are welcome to accompany their children to clinics held after regular school hours.
Students age 15 and older may fill out their own vaccination administration record (VAR) form.
Only the H1N1 flu vaccine will be administered, not the seasonal flu vaccine.
