 The Forest Grove School District will hold an H1N1 vaccination clinic on Tuesday, Nov. 10 at Neil Armstrong Middle School, located at 1777 Mt. View Lane, Forest Grove. The clinic will start at 4 p.m. and continue until the 1,000-dose vaccine allotment has been dispensed. Please do not line up for the clinic until after 3 p.m. when school is out for the day. Parking will not be available in the school parking lot until after 3 p.m.
There is no cost to receive an H1N1 vaccination. Both the nasal spray and shot versions of the vaccine will be offered. For information about the shot and nasal spray vaccine, visit the District web site at www.fgsd.k12.or.us. Children age 9 and younger will need two doses of the H1N1 vaccine, given about one month apart.
Anyone receiving an H1N1 vaccination must fill out a vaccination administration record (VAR) form. Forms will be available at the clinic and can also be picked up in advance at the district office, 1728 Main Street.
The clinic scheduled for Thursday, Nov. 12 at Cornelius Elementary School has been cancelled.
IMPORTANT NOTE: The Neil Armstrong clinic is limited to people in the following priority groups identified by the Centers For Disease Control: -Pregnant women -Children & young adults ages 6 months to 24 years -Health care workers -Emergency first responders, including police officers -Caregivers of children less than 6 months of age -Adults ages 25‐64 with underlying chronic medical conditions (diabetes, asthma, etc.). |
